| Coaching Application | Prior to the season, a potential coach is required to complete this document and email it to coaching@orillialakers.com along with supporting documentation. |
| Player Medical Information Sheet | Having your team’s medical sheets on hand can provide important information in the case of an emergency, as well as provide contact info for family physicians and emergency contacts when the player’s parents are unavailable. Coaches should also fill one out. |
| Lakers Tryout – Assessment Scorecard | This document can be used during tryouts by the coach for player assessment. |
| OBA Concussion Prevention Managment Policy | All coaches must abide by this policy. A concussion is a serious event, but you can recover fully from such an injury if the brain is given enough time to rest and recuperate. |
| Lakers Tournament Request Form | Before your team heads to a tournament or any game where you are required to pay to play, this form needs to be filled out and sent to the Treasurer. |
| Laker Venue Availability Form | When planning on hosting a game or tournament, this form needs to be filled out and sent to the Co-ordinator of Gymnasiums and Officials to check for availabilty of venues and officials. |
| Lakers Tournament Fee Request Form – When Hosting | Before your team hosts a tournament or game, this form needs to be filled out and sent to the Treasurer and Co-ordinator of Gymnasiums and Officials. |
| Laker Practice Time Request Form | If extra practice time is wanted outside of the scheduled time please fill out this form. Extra costs will be applied to the team budget. |
| Sponsorship Form | Complete for each team sponsor and submit with payment. |
| News Submissions | We are always excited to highlight the successes of our teams, athletes, and coaches throughout the season in the media. This document contains some tips and guidelines for preparing submissions for newspaper, magazine, and social media. |
| Rep Selection Opening Letter | This document can be used to inform players and parents of rep tryout details. Simply fill in as necessary. |
| Lakers Selected Congrats | This document can be used to officially inform a player that he/she has made the rep team. Simply fill in as necessary. |
| Lakers Not Selected | This document can be used to officially inform a player that he/she has not made the rep team. Simply fill in as necessary. |
| Parent Contract | This document highlights expectations towards parental behaviours and commitment. Parents will review, sign and return to you. |
| OBA Transfer Policy | If you have a player or coach/assistant coach that was in another Club the previous year, you will need notify the board so arrangements for payment of the $50 transfer fee can be made. |
